Care Coach Specialist

About the role

The Care Coach Specialist provides clinical assessment, education, and support to patients undergoing peritoneal dialysis, ensuring safe and effective treatment at home.

Key responsibilities

• Assess patients' clinical conditions and monitor overall health status.
• Perform and supervise peritoneal dialysis (PD) procedures and conduct home visits.
• Educate patients and caregivers on PD procedures, hygiene, supply storage, and adherence.
• Maintain accurate documentation, reporting, and update patient information using company software.
• Collaborate with physicians and multidisciplinary team members while adhering to ethical standards.

Required profile

• Bachelor’s degree in Nursing with at least 6 months of relevant experience, preferably in peritoneal dialysis.
• Fluency in Arabic and good command of English; French is an asset.
• Strong clinical assessment, patient‑centered communication, and problem‑solving abilities.
• Reliability, flexibility, and willingness to learn under clinical supervision.

Required skills

• Proficiency with digital platforms and healthcare software for patient documentation.
• Accurate documentation and reporting with strong attention to detail.
